Continuous Quality Improvement
The Pathways Community HUB Institute’s ® (PCHI®) model of care coordination, implement by the Harris County Pathways Community HUB (Harris County PCH), is a quality improvement framework for communities to build their own robust network of community-based care coordination in partnership with local stakeholders to align resources and achieve positive outcomes. The primary focus of the model is to improve health outcomes while addressing interrelated non-medical drivers of health such as, establishing safe housing, food security, education, and sustainable employment.
Measurement and quality improvement efforts are key to Harris County PCH as the work relies heavily on quality data to continuously make improvements, show impact, demonstrate the PCHI® Model’s efficacy to funders, and to spotlight resource deficiencies in our community. Being a part of the PCHI’s Network, data collected by Harris County PCH is shared with PCHI ® through a Quality Benchmark Report (QBR), which is a required data reporting tool for Certified Pathways Community HUBs. Quarterly reports are provided to PCHI®, and the data is aggregated to develop quality and performance reports that are used nationally.

Quality Improvement Methodology
Harris County PCH uses the Plan-Do-Study-Act (PDSA) method for rapidly testing a change within our system, by planning it, trying it, observing the results, and acting on what is learned. This is a scientific method used for action-oriented learning. After changes are thoroughly tested by using PDSA cycles, the information can be used to implement or spread change. The key principle behind the PDSA cycle is to test on a small scale and test quickly. The PDSA philosophy is to design a small test with limited impact that can be conducted quickly to work out unanticipated “barriers”.

Shalom joined NBHP in October of 2023. Shalom brings years of experience in the non-profit sector, with a focus in healthcare administration, data, and compliance. As our Quality Improvement and PCH Support Manager, Shalom works closely with our team of Community Health Workers, oversees data utilization, and leads program quality Shalom obtained his Bachelor’s degree in Psychology and Masters in Health Administration from Florida Atlantic University in Boca Raton, Florida.
