Director of Harris County Pathways Community HUB

Job Description: The Network of Behavioral Health Providers (NBHP) is a nonprofit organization comprised of the leadership of 40+ mental health and substance use disorder providers in the greater Houston area. Its mission is to improve delivery of, and access to, high-quality behavioral health services through education, collaboration, advocacy.
After more than a year of planning, NBHP is now poised to implement the evidence-based, pay-for-performance Pathways Community HUB (PCH) care coordination model in Harris County. The PCH contracts with Care Coordination Agencies (CCAs) to hire Community Health Workers (CHWs) who work to identify, assess, and connect high-risk clients with “Pathways” of medical, behavioral health, and social services in order to mitigate their risks. The Director of the Harris County PCH will be responsible for fully implementing this model, including cultivating and managing relationships with community partners, negotiating contracts, and supervising staff.

Compensation: $80 - $85K, commensurate with experience. Position offered as a full-time employee of NBHP (no benefits offered, but $6K allowance for health insurance purchase included as a stipend on top of salary).

Administration & Management
- Overseeing the day-to-day operations of the PCH.
- Supervising Harris County PCH Coordinator.
- Establishing PCH operational policies and procedures, including workflows, referral processes, and documentation.
- Managing the use of the PCH electronic platform by PCH staff, as well as CCA supervisors and CHWs.
- Planning and ensuring completion of required education and trainings for PCH Staff, CCA supervisors and CHWs.
- Receiving client referrals and assigning clients to CHWs to avoid duplication.
- Managing CHW caseloads to ensure they do not exceed established maximum limits.
Data Collection, Monitoring & Evaluation
- Monitoring CHW documentation and ensuring integrity of data entered into the PCH data platform.
- Consistently reviewing CCA performance outcomes to ensure continuous quality improvement.
- Tracking and reviewing Pathways completion.
- Reviewing compiled data and performance outcomes to determine the expansion to new populations and/or geographic regions.
- Providing monthly quality and outcomes reports to payers and CCAs.
Financial Management
- Negotiating contracts with health care payers.
- Reconciling and analyzing CCA supervisor and CHW documentation on a monthly basis to generate invoices to payers for completed Pathways, in alignment with negotiated contracts.
- Managing payments to CCAs in alignment with negotiated contracts.
- Consistently tracking revenue, expenses, in‐kind contributions, and reimbursements.
- Providing to the NBHP Executive Director administrative and financial statements for PCH-related activities, such as grant applications and reports.
Partnerships & Communications
- Managing relationships with contracted CCAs, payers, and referral partners.
- Being the “face” of the Harris County PCH in the community and presenting on the model at coalition, community meetings, etc.
- Engaging and recruiting new payers and CCAs to participate in the PCH.
Required Qualifications:
- Master’s degree in social work, nonprofit management, public health, or related field preferred.
- Deep understanding of the communities in Harris County, as well as knowledge of the general health, behavioral health, and social service landscape.
- Minimum five years of progressively responsible experience in nonprofit or health and human services leadership, administration, project management, and/or community engagement.
- Experience in management of program budgets and supervising staff.
- Excellent organizational and coordinating skills, including strong attention to detail and a dedication to quality and accuracy.
- Excellent verbal and written communication skills.
- Ability to juggle multiple responsibilities and meet strict deadlines.
- Capacity to think strategically and ability to work collaboratively with persons from diverse disciplines and with different points of view.
- Flexibility in dealing with changing demands.
- Self-starter, with an ability to fulfill job requirements with minimal supervision.
- Competency in Microsoft Office (Word/Excel/Powerpoint/Publisher, etc.) and Google Suite (Google Docs/Slides/Sheets, etc.).
Preferred Qualifications
- Previous work with or knowledge of the Pathways Community HUB model.
- Experience working with health care payers and/or expertise in health care reimbursement systems.
- Working knowledge of basic evaluation methods and approaches.
- Experience in grant researching and/or grant writing.
