Harris County Pathways Community HUB - Community Advisory Council
The Pathways HUB Community Advisory Board provides guidance on the direction, strategies, and quality improvement activities of the Harris County Pathways Community HUB (PCH) to support individuals in need of care coordination and related services. The advisory board includes representatives from diverse organizations and stakeholders who understand and can best represent the clients served by the HUB, including, but not limited to: Care Coordination Agencies, Community Health Workers, HUB clients, health systems, payers, city and local government, public health, social service providers, businesses, philanthropy, and other community organizations.
