This Year's Harris County Pathways Community HUB Program Champion Award:
Honoring Debbie Levin, LCSW at Memorial Hermann Hospital, Southwest Debbie's Bio, in her own words: "I have over 30 years’ experience as a hospital social worker, specializing in women’s and children’s services. I began my career at the University of Texas Medical Branch where I worked for 2 decades. I was also a social worker at Texas Children’s for several years, in the hospital and in their community clinics. It was at Texas Children’s Pediatrics (TCP) Cullen where I connected in a very meaningful and direct way with community resources and that passion for community connectedness has remained in my heart. I assumed leadership positions at Texas Children’s as a senior social worker for TCP and at Texas Children’s Health Plan as the Manager of Behavioral Health and Complex Care Case Management for Medicaid and CHIP. Much of my career at Texas Children’s focused on behavioral health and social determinants of health, reducing health disparities and linking patients and families directly to care and community resources. For the past 3 ½ years, I have been the Mother/Baby social worker at Memorial Hermann Southwest. In this role, I have developed many new connections with community agencies in order to help the women and families I serve gain and receive access to services with the goal of reducing maternity morbidity and mortality and helping generations to come. I can contribute the knowledge of perinatal social work and hospital processes, as well as the benefits of providing direct linkage to community resources and “warm handoffs.” I can also attest to the importance of assessing and addressing women’s mental health, substance use, trauma history, safety and social determinants of health during pregnancy and at delivery in order to not only help the patient, but the infant, her family, and generations to come." |
This Year's WREDI (Workforce, Recruitment, Education, & Development Initiative) Program Champion Award:
Honoring Diane Arms, Director of Community Behavioral Health at The Council on Recovery and oversees the Center for Co-Occurring Disorders. Diane Arms currently serves as the Director of Community Behavioral Health at The Council on Recovery and oversees the Center for Co-Occurring Disorders. She has dedicated her career to serving the Latino population in the Health Field, including Mental Health and Substance Use. She received both her Bachelor of Arts and Master of Arts Degree in Clinical Psychology from the University of Texas at El Paso. Diane has served in roles such as Director of Health Integration and Director of Prevention and Counseling. She has worked facilitating leadership meetings amongst local Federally Qualified Health Centers to proactively identify, address, and resolve systemic issues within the healthcare system, has successfully implemented programs such as Multisystemic Therapy in a Mental Health Setting and Transition Age Youth, assisting transitioning clients from children’ services to adult services in the mental health system. She spearheaded the transition of in person clinical and behavioral health services of a healthcare agency to telehealth services to accommodate social distancing and stay at home orders due to COVID 19. Previously funded projects consist of identifying relationships between stress, depression and anxiety to substance use and parenting styles in young adult Latinos, investigating the effects of chronic illnesses on children’s health-related quality of life in the Colonias, an underserved and impoverished neighborhood along the Texas-Mexico border, and exploring effective decision making aids on colorectal cancer for the aging Latino population. She is a fellow of the American Leadership Forum, Class 44, a graduate of the Mujers de HACE program, 2019, a board member for El Centro de Corazon and a Consultant and Trainer for Hispanic/Latino Behavioral Health Center of Excellence. |
