Community Health Workers
Community Health Workers (CHWs) play an integral role in the success of Harris County Pathways Community HUB. CHWs are on the frontline, providing culturally and linguistically appropriate health education, and participant care coordination to achieve health equity. Our CHW team works collaboratively across various sectors of care to address non-medical drivers of health (NMDoH) that impact vulnerable populations, such as pregnant and post-partum women, as well as individuals who are experiencing behavioral health challenges. Community Health Worker interventions can result in cost-savings for organizations by eliminating barriers to care and shifting participant care experiences from expensive urgent or emergency care to more cost-effective primary and preventive care. |
Meet the Harris County Pathways Community HUB CHW Team:

Melinda has a degree in Social Work and is currently working with the City of Houston Health Department as a CHW in the Harris County PCH Program. She has been with the Health Department now going on 10 years in October. She is passionate about helping families, children, and especially single mothers. "I love what I do and hope to continue for as long as I’m able."

Kadiedra has a Community Health Worker with the City of Houston Health Department, serving the community for over 10 years throughout the Houston area and beyond. She has experience providing in home/group education as a Lactation Consultant to over 65 expectant mothers, having successful outcomes. Some of her strengths include versatility, flexibility, and creativity along with hard work. These strengths have yielded her the exposure, experience, and pleasure in various fields of administration, procurement, logistics, finance, and quality assurance.

Daphane is a dedicated community health worker with experience since 2021, working at Santa Maria Hostel. She is committed to supporting individuals in need and those seeking additional assistance, working diligently to ensure her participants' needs are met.

Jonathan is a dedicated professional in the field of community health, working with a non-profit organization that supports individuals struggling with addiction. As part of a team of four Community Health Workers, Jonathan is assigned to one of Harris County's community centers, which collectively serve precincts 1, 2, 3, and 4. Jonathan emphasizes the importance of providing clients with clear guidance to navigate complex systems such as enrolling in the Gold Card program, Medicaid, private insurance, and other critical services. Additionally, he assists individuals in accessing resources like food programs, scheduling medical appointments, finding low-cost healthcare providers, and other essential activities.
“We serve as a navigational aid for individuals who may feel disoriented in the application process for resources,” Jonathan explains, underscoring the center’s commitment to empowering clients to achieve their goals.

Lisa is a Community Health Care Worker and Supervisor with dual certifications as a Mental Health Peer Support Specialist and Peer Recovery Support Specialist. Her professional background includes working in criminal justice settings with people who have substance use and mental health issues and with clients diverted by law enforcement for public intoxication to Houston Recovery Center's Sobering Center. Lisa brings lived experience from her recovery journey to client interactions, guiding them by example and coaching them to apply new coping skills as they improve their life and well-being.
As a CHW, Lisa established positive working relationships with stakeholders across Harris County Precincts by conducting outreach and fostering health equity for communities with limited access to care that COVID-19 compromised. She specializes in delivering care coordination to facilitate client engagement in community activities and service placement, following-up through completion, and helping people regain social resources to rebuild their lives. Lisa is an encourager, a resource broker, and a guide for people who can benefit from receiving a leg up to stay on track in life.

Dinnkia has been a community health worker for two years, supporting individuals from diverse backgrounds and life experiences. She is particularly passionate about Mental Health First Aid for both children and adults, as well as behavioral health, and holds certifications in all three areas. Known for her hardworking nature, excellent communication skills, and strong dedication to achieving goals, Dinnkia brings a wide range of strengths to her work. She truly enjoys her role and remains eager to expand her knowledge and skills within the field.
