Harris County Pathways Community HUB Program
​Community Health Workers 

Community Health Workers (CHWs) play an integral role in the success of Harris County Pathways Community HUB. CHWs are on the frontline, providing culturally and linguistically appropriate health education, and participant care coordination to achieve health equity.  Our CHW team works collaboratively across various sectors of care to address non-medical drivers of health (NMDoH) that impact vulnerable populations, such as pregnant and post-partum women, as well as individuals who are experiencing behavioral health challenges. Community Health Worker interventions can result in cost-savings for organizations by eliminating barriers to care and shifting participant care experiences from expensive urgent or emergency care to more cost-effective primary and preventive care.
 

Meet the Harris County Pathways Community HUB CHW Team:

PictureTianca Gilliard, CHW, The Council on Recovery
Tianca is a mother of two and a resident of the Sunnyside/South Park community. Tianca has been a certified community health worker going on five years and has been great with engaging with community partners as well as many residents throughout Harris County. She has trained as a birth doula and is a maternal health/community activist. Tianca is outstanding with advocating and being the voice of the community when it is needed. Tianca is currently working as a Community Care Coordinator at the Council on Recovery for Pathways. Her role requires her to engage with participants on a one-on-one basis to meet them where they are in order to provide the personal support that they may need. While maintaining a work/life balance Tianca still strives to bring awareness to the injustices within our health system especially those centering on maternal and mental health.

Hello, my name is Stephanie Bialobos. I’ve been a community health worker for a little over a year, and I love helping the community in any way possible. I have been able to help people to get connected to the resources they need, and it is so rewarding. I’m looking forward to many more years of community health work.

My name is Melinda Rodriguez, I have a degree in Social Work and am currently working with the City of Houston Health Department as a CHW in the Harris County PCH Program.  I’ve been with the Health Department now going on 10 years in October. I am passionate about helping families, children, and especially single mothers. I love what I do and hope to continue for as long as I’m able.

Hello, I am Kadiedra Jenkins and I have worked with the City of Houston Health Department, serving the community for over 10 years throughout the Houston area and beyond.  I’ve performed in home/group education as a Lactation Consultant to over 65 expectant mothers, having successful outcomes.  Some of my strengths include versatility, flexibility, and creativity along with hard work. These strengths have yielded me the exposure, experience, and pleasure in various fields of administration, procurement, logistics, finance, and quality assurance.

Nicol Smith is currently employed by Santa Maria Hostel as a Recovery Support Peer Specialist, Mental Health Peer Specialist and Community Health Worker. Nicol has worked with a variety of populations, including pregnant and postpartum, women who have criminal justice involvement, and families involved with DFPS. She has dedicated her days to advocating for substance use disorder, mental health, and trauma informed care and looks forward to continuing to empower women and their families to live healthy, successful, productive and self-fulfilling lives.
